I authorize Hendersonville Animal Hospital to release medical information on my pet(s) to another veterinary clinic, Spouse/Co-Owner, local law enforcement (e.g.: in the case of rabies vaccination verification), or boarding facilities/groomers requesting vaccination and laboratory history.

Payment Policy

Payment is expected when services are rendered. In an effort to provide as many payment options as possible, we accept the following in any combination: Cash, Visa, MasterCard, American Express, Discover, CareCredit, and Scratch Pay. Hendersonville Veterinary Hospital does not extend any form of credit. A late charge of 1 ½ % (18% annual) is charged on all past due amounts. Outstanding balances over 30 days may be referred to collections.
